CHAPTER 1

Introduction to Ophthalmic Surgery

C. Robert Bernardino

Ophthalmic surgery encompasses any surgical procedure performed around, on, or in the eye. This periocular region is fairly small defined by the bony orbit, soft tissues contained in the orbit, and the eyelids, which border its anterior-most portion. Within these confines, surgery ranges from basic eyelid reconstruction to the most refined and delicate retinal surgery; techniques have been developed that typically use magnification and specialized instrumentation to work around this small field. Basic knowledge of the anatomy, instrumentation, and surgical technique will go a long way for the medical student, technician, or resident early in his or her career to be an active participant during these complex procedures. Failure to be prepared for surgery relegates one’s role to observer in the best scenario and liability in the worse case.

This book is designed to give an overview of the different surgical procedures performed by an ophthalmologist. The basic anatomy will be reviewed. Important pathologic scenarios will be described. The procedures will be delineated in a step-wise fashion. A high-yield review section is included with each surgical procedure, which includes surgical pearls. This guide should serve as the first resource in learning about ophthalmic surgical procedures, a review of principles prior to surgery, and a concise summary for postsurgical analysis and critique.
