Prophylactic Selective Laser Trabeculoplasty in the Prevention of Intraocular Pressure Elevation After Intravitreal Triamcinolone Acetonide Injection




In the article titled “Prophylactic Selective Laser Trabeculoplasty in the Prevention of Intraocular Pressure Elevation After Intravitreal Triamcinolone Acetonide Injection,” by Bozkurt and associates, the authors mentioned that the intraocular pressure (IOP) elevation after intravitreal triamcinolone acetonide injection may be prevented by performing selective laser trabeculoplasty (SLT) before the injection, especially in cases with a baseline IOP of 21 mm Hg or more. This study provides a valuable contribution to the literature since glaucoma has been one of the most common problems after intravitreal triamcinolone injection. Furthermore, baseline IOP was reported to be a risk factor for increased IOP after the intravitreal triamcinolone injection. Applying SLT to these patients before the procedure might be practical and easy way to solve this problem to some extent.


In the current study, patients with a history of glaucoma were reported to be excluded. Mean central corneal thickness and cup-to-disc ratio values were found to be comparable. On the other hand, the authors did not give any data regarding how they excluded the patients with newly diagnosed glaucoma. In our opinion, the results of the study would be more conclusive if the authors had also performed retinal nerve fiber layer (RNFL) analysis or visual field test in the study group. Patients with ≥21 mm Hg baseline IOP might have a newly diagnosed glaucoma if associated with glaucomatous RNFL or visual field defect. Questions remain as to whether the study group was homogenous or not. Meticulous discrimination of these patients is of importance in that IOP response and need for additional SLT or topical antiglaucomatous therapy might have been different in cases with glaucoma and without glaucoma.
