V aisman et al reported a 93% complete response rate with topical interferon-alpha2a for primary acquired melanosis with atypia over a follow-up exceeding 10 years. Duration is more important than the response rate. Short-term responses in patients with PAM have been reported in numerous small retrospective series; however, data showing durable disease control over a decade in a condition that can progress to conjunctival melanoma are harder to come by.
The safety profile is another clinically relevant finding. Mitomycin C remains the main comparator, and its ocular surface toxicity limbal stem cell deficiency in particular is a genuine problem, especially in patients with diffuse or multifocal disease, in whom repeated or prolonged treatment is needed. Largely mild and reversible adverse effects with IFN-α2a over extended administration is a meaningful difference, not just a favorable footnote.
The question of whether to use IFN-α2a or IFN-α2b has practical significance. IFN-α2b has more published history in ophthalmic oncology, but supply constraints are real in some regions. Comparable outcomes with IFN-α2a mean that clinicians facing a shortage have an evidence-based option rather than an improvised one.
Heterogeneous treatment history is the main interpretive problem. Patients had prior surgical excision, mitomycin C, and plaque brachytherapy in varying combinations before IFN-α2a. Isolating what interferon contributed to disease control in this context is not straightforward, and the 93% figure reflects the full clinical picture rather than interferon therapy alone.
The treatment duration ranged from 1 to 10 months without clear criteria, which limited reproducibility. Whether the variation was driven by disease extent, response speed, tolerability, or clinical judgment is not documented, and this is important for anyone trying to apply these results systematically. Standardized dosing and duration criteria are prerequisites for this approach to become a defined protocol rather than an individualized practice.
