INTRODUCTION
Carbon dioxide therapy (CDT) is a procedure which is very similar to mesotherapy. It consists of intradermal or subcutaneous injections of medical grade CO 2 using 30 G needles. With a few medical applications published in the medical literature, CO 2 insufflation has been widely used in the last 8 years in South America and in particular in Brazil for cosmetic purposes, such as stretch marks, cellulite, localized adiposities and under-eye dark circles, in combination with other minimally invasive aesthetic procedures. The low morbidity, high level of patient tolerance and minimal side effects makes it a very interesting technique in our battery of face and body rejuvenation techniques, with a high level of patient satisfaction.
CDT (also known as carboxitherapy, carboxytherapy, carbossitherapy or carbotherapy) was first described in the central mountains of France in the early 1930s, in the medical spa of Royat. Doctors found that transdermal use of CO 2 in enriched warm baths improved vascular ischemia symptoms because of a strong direct vasodilator effect, a neo-angiogenesis effect and better oxygen release in superficial tissues related to the Bohr effect (see below). Authors soon tried subcutaneous injections of the spa’s CO 2 gas, looking to find optimized and stronger effects.
There are numerous available PubMed and MedLine articles about vascular uses of injected CO 2 . Many hundreds of thousands patients have now been treated, with indications such as lower limb ischemic arterial pathology, Raynaud’s phenomenon, skin ulcers with arterial or venous cause, as well as several vascular erection disorders.
The aesthetic use of CO 2 insufflations is relatively new, the first articles being published in around 1990 by Belotti and De Bernardi and around 2000 in Italy. These articles concerned improvement of localized adiposities with CDT, the treatment sometimes being associated with liposuction. The number of available articles on the subject is limited to the two main studies of D’Aniello and Brandi from the University of Siena.
Despite the lack of any publication in other indications, CDT is also commonly used in the following procedures:
