The questionnaire provided by Gianoli and Soileau of The Ear and Balance Institute, in addition to providing essential questions, is useful for physicians evaluating the litigating patient presenting with dizziness. The questionnaire provided by McCaslin and colleagues of Vanderbilt University is suitable for children and adults. O’Reilly and colleagues of Alfred I. duPont Children’s Hospital provide useful questions appropriate for pediatric patients.
